The Association’s Conduct Committee may participate in resolving disputes between patients and member optometrists. The Committee comprises three optometrists who are members of the Association’s Council.

Please note that all complaints must be lodged in written form via email, fax or post.

The following details must be supplied:

• the Optometrist’s name;
• if the Optometrist’s name is not available, the name of the practice where treatment took place;
• the date of treatment;
• the type of treatment received;
• the nature of the complaint;
• the complainant’s name, contact address and phone number.
